The USOTC Paperwork is due by January 7th, 2011 and should be sent to:

Gymnasts and Coaches cannot participate if the paperwork is not received by January 7, 2011. Important reminder – all paperwork must be signed and dated in 2011(example – January 1, 2011 or after is acceptable). No copies of a signature are acceptable.

1. There is no official entry form since every team has their own form. Please include:
   1. All coaches names and e-mail addresses.
   2. All coaches USAG #'s - safety expiration - background expiration.
   3. All coaches professional number expiration date/s.
   4. There will be a special coach's gift for each coach so please let us know the exact number attending.
   5. Main contact info for team, including name and phone number.
   6. Address of gym.
2. There is a waiver form that must be signed by all athletes, coaches and officials. This form will be good for the 2011 year.
   
3. Make sure your birthday is accurate and written on the bio information sheet.
4. Coaches do not fill out the medical portion of the waiver.
